What should I bring? For the onsite clinics, employees need to bring an Informed Consent Form AND either their Coventry card (if covered on the County health plan) or other County ID (if not covered on the County health plan), like an employee or school ID or paystub.

Spouses, dependents and retirees who are covered on the County health plan are eligible to receive a vaccination at a pharmacy. Giant pharmacists are able to vaccinate children aged 6 and up, but other pharmacies may have different policies. Learn more »
